Inspiration in Action: Bridgepoint Active Health 2013-2014 Annual Report

Chronicling a year of embracing change and bringing active healthcare to life – which has skyrocketed to the top of Toronto’s acute care referral list – Inspiration in Action provides a snapshot of Bridgepoint’s first year in our purpose-built complexity hospital.

The report features personal stories of patients that demonstrate the progress being made to transform patient care through our active healthcare model, groundbreaking research and educational programs – and illustrates how we are getting patients with complex health conditions back to their lives.

“I will never forget the first thing they had me do when I arrived at Bridgepoint. They asked if I had my own clothes and if I wanted to put them on. I was so thrilled to get out of my blue hospital gown! That was the first step to getting back my dignity and my life,” says Lisa Niblett, a former Bridgepoint patient, who arrived at the hospital unable to stand on her own. Lisa is now walking again and working towards regaining her driver’s license.

Our leading-edge, applied research is informing both clinical practice and big-picture system-wide change. The hospital provides a living laboratory for scientists from our Collaboratory for Research and Innovation to look at complexity from three distinct perspectives: patient, care providers, and physical environment. We are also training the complexity experts of the future – with over 640 students learning our interdisciplinary model of care last year.

Our full report provides many more inspiring stories of how our care model, research and community partnerships are improving the outcomes for patients living with complex health conditions.
