The focus of the Bridgepoint Collaboratory for Research and Innovation is health system innovation in complex chronic disease prevention, management and care. 
  • Health services and policy research is the core area of expertise supplemented by clinical and population health research.
  • The work will examine the fundamentals of prevention and care (patient-oriented research about quality) in complex chronic disease.
  • Each of the theme areas will include health informatics and metrics, intervention and health outcomes research, and knowledge translation, including health economics and policy. Each theme will have an international linkage, and is flexible in terms of content, linkages among themes, and relative size.
  • Understanding complex chronic disease and its determinants will be an umbrella theme.
  • Each team combines clinical, administrative and research expertise, including policymakers and patient/family members.
  • A post doctoral fellow and at least one Bridgepoint Health lead will be included on each inaugural theme.

Excellence in Complex Chronic Disease Quality and Safety
What are the fundamentals of care in our patient population? What are the simple, complicated and complex questions that need to be addressed in complex chronic disease clinical practice? What works with whom and under what conditions? What are the best approaches in continuity of care with these patients?
