When your physician is not available, you may be seen by another primary care provider for urgent, same-day issues. Please schedule your routine appointments with your physician so they can provide continuity in your care.
After-Hours Urgent Care Clinic
Our After-Hours Urgent Care Clinic is available to all patients of the Bridgepoint Family Health Team who need urgent care due to the sudden onset of symptoms. This is not a walk-in clinic. Patients must pre-register for an appointment during the times noted below to be seen by a member of the care team. In order for the Bridgepoint Family Health Team to ensure continuity of care is maintained, we ask that you try to avoid going to a walk-in clinic and access your care through our Advanced Access services and After-Hours Clinic.
After-hours care is available on:
- Mondays, Tuesdays, Wednesdays and Thursdays from 5 p.m. to 8 p.m. – the last patient is registered at 7:45 p.m.
- Saturday mornings from 9 a.m. to 12 noon for urgent care – the last patient is registered at 11:45 a.m.
If you require non-urgent care please call during regular daytime office hours to book an appointment.
Prescription refills, pre-op exams, or form completions are not provided at the After-Hours Clinic.
Office Policies
*Please do not go to a walk in clinic… call us first!
- You must bring your OHIP card and all medications with you to each visit.
- Call to schedule an appointment when needed. We offer same day or next day access.
- Most, but not all, of your appointments will be with your usual primary care provider. You may be seen by a different doctor or by a nurse practitioner under special circumstances (e.g., vacation, urgent problems needing immediate care, chronic disease management).
- Prescriptions will only be filled at an appointment. We will give you enough medication to last until your next appointment.
- We receive lab results from Canadian Medical Laboratory (CML) electronically. To speed the process of receiving test results, it is strongly suggested that you use a CML lab.
- If labs results are abnormal and you need follow up for this, the office will contact you.
- We encourage family members to be seen by the same primary care provider.
- No narcotic or other addictive drugs will be prescribed on the first visit, and will not be prescribed until a full assessment has been done and old records obtained.
Advanced Access Scheduling System
What is Advanced Access?
Advanced Access is a model of scheduling appointments within 1-2 working days from when an appointment is requested.
Why Advanced Access?
There is good evidence that this model benefits both patients and physicians by:
- Giving patients increased access to their physician;
- Greater flexibility of appointment times available;
- Giving physicians the ability to see their patients when they need to be seen;
- Decreasing a backlog of appointments;
- Decreasing missed appointments.
How Does Advanced Access Work?
Call to schedule an appointment when you need to be seen.
This includes:
- Routine follow-up (physician has asked you to return after a specified time – i.e., two weeks, three months, etc.).
- Preventive care (i.e., annual physical).
- Non-urgent issues that arise (i.e., health education, immunizations, etc.).
You will be asked if you would like an appointment that day or the following day.
Urgent care (sore throats, earache, fever, etc.) appointments will always be scheduled the same day you call.
We also have After-Hours Clinics for urgent care by appointment only – Monday/Wednesday/Thursday 5 p.m. - 8 p.m. and Saturday 9 a.m.-12 noon. This is not a walk-in clinic – you must call in advance for an appointment.

Patient Privacy
Your physician and the Bridgepoint Family Health Team staff are bound by law and ethics to safeguard your privacy and the confidentiality of your personal information.
This includes:
- Collecting only the information that may be necessary for your care;
- Keeping accurate and up-to-date records;
- Safeguarding the medical records in our possession;
- Sharing information with other health care providers and organizations on a “need to know” basis where required for your health care;and
- Disclosing information to third parties only with your express consent, or when necessary for legal reasons, and retaining/destroying records in accordance with the law.
Your request for care implies consent for our collection, use and disclosure of your personal information for purposes related to your care. As noted above, other purposes require your express consent.
You have the right to see your records. You may also obtain copies of your records (see the receptionist for our fees for this service). Please speak to your physician if you have concerns about the accuracy of your records.
If you would like to discuss our privacy policy in more detail, or have specific questions or complaints about how your information is handled, please speak to your physician and he or she will try to resolve them.
