Welcome New Employees

Welcome to Sinai Health! We are delighted you have chosen to build your career with us and we look forward to you joining our organization.

Below you will find instructions on items to complete prior to your start date and details about what to expect on your first day.

Items to complete prior to your first day

To ensure a smooth onboarding experience, please follow the two steps below and complete the required items before your first day.  

Step One – Occupational Health Requirements

Due: No later than 12 p.m. on the Wednesday before your start date

  1. Immunization and Surveillance Form

    Sinai Health’s Immunization and Surveillance Policy ensures all Sinai Health people have the immunizations necessary to minimize the risk of exposure and possible transmission of communicable diseases to our people and patients.

    You must receive immunization clearance prior to your scheduled start date. You will not be permitted to start work nor attend Orientation until you are cleared by Occupational Health.

    The testing and immunization documentation required takes time to complete, possibly up to several weeks. It is important that you begin this process and book an appointment with your primary care provider as soon as possible.
  2. Employee Health Review Questionnaire

    Complete and return the Employee Health Review Questionnaire to Occupational Health by fax (416-470-6725) or email (ohs.hbh@sinaihealth.ca), no later than 12 p.m. on the Wednesday before your start date.
  3. Proof of N95 Mask Fit Testing (if applicable)

    If you have been fit tested for an N95 mask in the last two years, please send a photo or scan of your card indicating the type of mask you were fit tested for to N95.MaskFit@sinaihealth.ca.

    If you require an N95 Mask Fit Test for your role and have not provided proof of a fit test within the last two years or if we do not have the specific mask you were fit tested for, you will be scheduled to attend a Mask Fit Testing session on your first day. Please watch for an email from N95.MaskFit@sinaihealth.ca.

Step Two - New Employee Documentation

Due: On your first day

Please bring your completed New Employee Documentation Package and supporting documentation, as outlined on page one, with you on your first day.
