The Bridgepoint Health Alternative Level of Care (ALC) Connect Program is an interprofessional service designed to effectively network medically stable patients in acute care to the most appropriate post acute care environment.
The goal of the program is to ensure smooth transitioning and integrated movement of the patient to the most appropriate services at Bridgepoint, and integrated movement across the continuum to the most appropriate level of care.
Our ALC Connect patients:- benefit from a moderately paced rehabilitation therapy service from an interprofessional team as a result of an injury/surgery, prolonged acute care stay;
- have the potential for improvement in functional abilities as a result of the rehabilitation;
- are not dependent on active medical treatment from an acute care hospital;
- have identified some realistic attainable functional goals;
- require nursing care to meet activities of daily living on a 24-hour-a-day basis; and
- must be able to travel and transfer to an outpatient dialysis program if receiving dialysis.

With significant experience in detailed and specialized assessments that facilitate care plan development, treatment and triaging, Bridgepoint's ALC Connect team has expertise in assessment, pain and wound management, and symptom control.
