“Who are our People?” The Characteristics, Needs and Experiences of the In-Patient Population at Bridgepoint Health


On Tuesday, May 15, Kerry Kuluski, PhD Research Scientist, Bridgepoint Collaboratory for Research and Innovation, Assistant Professor (status), Faculty of Medicine, University of Toronto presents: Across the world, a growing number of individuals are living longer with complex chronic disease which is commonly characterized by multiple health problems, emotional challenges and heavy reliance on health system resources. This presentation provides an overview of the results of the Bridgepoint Study; groundbreaking research on patients with complex chronic disease. Findings illustrate the challenges experienced by patients and conclude with ways to optimize the patient experience through refinements in data collection and methods of care delivery.

The Health Coach is a new profession for “keeping more people healthy longer."
 

Reconciling the Fundamentals of Care with Patient Centred Care
 - Alison Kitson, Head of School of Nursing at the University of Adelaide, discussed the mounting concern in health systems around the globe that the fundamentals of care are being neglected. There is an implicit view that providing the fundamentals of care in hospital is a straightforward thing to do and policy initiatives such as patient centred care are being developed. This presentation explores these assumptions using a study to illustrate challenges facing healthcare professionals, health systems policy makers and patients.
 
Managing People with Multiple Chronic Diseases: A Global Response in the Age of Social Media - Dr. Alex Jadad, Chief Innovator and Founder of Global eHealth Innovation and Canada Research Chair in eHealth Innovation 
 
Patterns of Health Care Use by an Aging Patient Population - Dr. Ross Upshur, Canada Research Chair in Primary Care, discusses the results and implications of two recently published studies looking at medication and health service usage by patients over the age of 65 in the province of Ontario.
 
ELLICSR Cancer Survivorship Program - Dr. Joyce Nyhof-Young, an Education Research Scientist with the Cancer Survivorship Program at Princess Margaret Hospital, discussed the Electronic Living Laboratory for Interdisciplinary Cancer Survivorship Research (ELLICSR). This library has been developed to study an innovative survivorship research strategy called a “Community of Survivors.”
